Fractured IVC Filter Fragments: The Penn Perspective

September 23, 2026 · 5 min 32 sec

Show notes

This episode features expert commentary from Dr. Scott Trerotola of Penn Medicine, responding to our recent episode on Stanford's fractured IVC filter registry titled, Fractured IVC Filter Fragments: When to Chase the Metal.

  • Shared Ceiling: Both the Penn and Stanford series found that, at best, about half of patients leave with every fragment removed. Some extravascular fragments can still be reached by direct CT- or fluoro-guided puncture.
  • Pulmonary Is Not a Parking Spot: Dr. Trerotola argues that a fragment moving from the heart into the lungs is not a success. Fragments can be coughed up or reach the systemic circulation, and removal is low risk in experienced hands.
  • Radiation in Perspective: Penn's median fluoroscopy time was 27 minutes, so dose should not drive the decision.

Tune in to hear why a fragment in the lung is still worth chasing.
